                Makati Health Certificate Application

1. Check your email for an invite from the Makati City support team, then click the website link.
   
   
   
2. Then you will be redirected to the Makati Health Cert web page. Log in using the credentials provided in the email.
   
   
   
3. After logging in, read the Data Privacy Agreement. Then, click the checkbox for 'I have read and agreed to these terms and conditions' and click 'Close'
   
   
   
4. After that update your password, then click 'Submit'. 
   
   Note:
   It would send a email confirmation for the change of password.
   
   
5. You will be redirected to the login page again. Log in to your account.
   
   
6. Click on 'Edit' then complete your Personal information or the details that has an '*' to proceed, then Save.
   
   
   
7.  On the bottom part for the Employment Information, click the button for 'Action'
   
   
   
8. Choose the category 'Non-Food Handler', then select the position 'Office Personnel & Staff'. Then click 'Submit'
   
   
9. After that, go to the Health Certificate Application tab and complete the remaining required details. Then, under Employment Information, select 'VIATECHNIK INC.' and proceed to click 'Submit.'
   
   
   
10. After submitting, you will be redirected to the Seminar tab. Once you complete the seminar, you will be prompted that you have passed. Click 'OK' to proceed.
    
    
    
11. After that, you will be redirected to the Payment tab. Complete the payment using either cash or online payment. Instructions for both payment methods will be provided. Once done with the payment complete the details needed from the transaction the click 'Save'.
